Registration Deadline: March 15, 2009
All registration forms and final payments must be received by March 15, 2009. Any registration forms or final payments received after the registration deadline will be charged a $10 per performer late fee.

Please duplicate the following forms for your dancers and their families. These documents include all information the studio director requires to register their studio to attend the Diamond Dance Spectacular.
