Registration Deadline: December 31, 2008
All registration forms and final payments must be received by December 31, 2008. Any registration forms or final payments received after the registration deadline will be charged a $10 per performer late fee.

Please duplicate the following forms for your dancers and their families. These documents include all information the studio director requires to register their studio to attend the Crystal Dance Spectacular.

ALL DIRECTORS MUST SUBMIT an Audition Request form along with a audition tape/dvd to be considered for the showcase performance.
